Medical Prescription Insurance
- Choice between two plans:
- Health Partners PPO (Copay Plan)
- Health Partners HDHP (High Deductible Health Plan)
- Both plans are partially subsidized by NGHS.
- The Health Partners PPO Plan provides pharmacy co-pays according to the choice of generic, preferred and non-preferred brand prescriptions.
- Employee Pharmacy is available to all NGHS employees.
- Coverage is available to full-time and part-time employees.
- Coverage begins on your date of hire.
Dental Insurance
- Preventive, basic and major services included.
- Orthodontic services are available for children under 19.
- May visit any dentist.
- Coverage available to full-time and part-time employees.
- Coverage begins on your date of hire.
Vision Insurance
- Choice between two networks.
- Co-pays for exam and materials.
- Coverage available to full-time and part-time employees.
- Coverage begins on the 1st of the month following your date of hire.
Life/Accidental Death Insurance
- Coverage available to full-time employees, premiums fully paid by NGHS.
- Optional additional Life & Accidental Death coverage.
- Optional Spouse & Dependent Life Insurance.
- Coverage begins on the 1st of the month following your date of hire.
401(k) Retirement Savings Plan
- Available to all employees, age 18 or older.
- Opportunity to contribute up to the maximum annual amount set by the IRS.
- Employees automatically enrolled at 3% of earnings; the ability to opt-out provided.
- NGHS will match a portion of the employee’s contribution.
- Several investment funds to choose from.
- Coverage begins on the 1st of the month following 30 days of employment.
Flexible Spending Account
- Pre-tax money set aside in Health and/or Dependent Care accounts for future reimbursement.
- Coverage is available for full-time and part-time employees.
- Coverage begins on the 1st of the month following your date of hire.
Short-term Disability
- Elect coverage up to 60% of gross weekly income.
- Price based on the level of coverage selected, up to $5,000 in weekly benefits.
- Coverage is available to full-time and part-time employees.
- Coverage begins on the 1st of the month following your date of hire.
Long-term Disability
- Coverage is available to full-time employees.
- The benefit pays 60% of your gross monthly income with a maximum monthly benefit of $10,000, premiums fully paid by NGHS.
- Coverage begins on the 1st of the month following your date of hire.
Health Savings Account
- An employee can choose the amount to be deducted each period, up to the maximum allowed by IRS for employees enrolled in the Health Partners HDHP Plan.
- Tax-free funds.
- NGHS may contribute to account-based wellness plan participation and on the tier of medical coverage.
- Coverage begins on the 1st day of the 2nd month following your date of hire.
Unum
- Supplemental insurance policies include, accident, critical illness, and hospital protection.
- Depending on the plan selected, premiums are payroll deducted on a pre-tax or after-tax basis.
- Coverage is available to full-time and part-time employees.
- Coverage begins on the 1st of the month following your date of hire.
Legal
- Provides access to professional legal representation.
- Coverage begins on the 1st of the month following your date of hire.
Paid Time Off (PTO)
- Automatic accrual for full-time and part-time employees upon 1st day of employment
- Includes vacation, holiday, sick and personal days.
- Opportunity to cash in PTO hours one time each year, up to 80 hours. Payment is made based on the employee’s current base rate of pay.
- Available to full-time and part-time employees.
Continuous Learning Opportunities
- Tuition reimbursement for full-time and part-time employees: Maximum of $5,250 per calendar year up to a lifetime maximum of $21,000 for relevant degree programs.
- In-service educational courses are offered by NGHS.
